Copies of Exams
Copies of radiology exams performed at Northwest Community Hospital can be obtained at the Patient Services Center Monday-Friday from 8 a.m. to 8 p.m., and on Saturdays and Sundays from 9 a.m. to 4 p.m. Patients should call 847-618-3200 a minimum of 24 to 48 hours before they need their exam copies to ensure their materials will be ready.
To get a copy of your radiology exams/records, you must fill out and return a Records Release Statement.
